Acknowledgment of Risk
I understand that participation in fitness training, wellness coaching, exercise programs, and/or physical activity involves inherent risks, including but not limited to:
- Muscle soreness
- Sprains or strains
- Increased heart rate or blood pressure
- Injuries resulting from equipment use
- Aggravation of pre-existing conditions
- In rare cases, serious medical events (e.g., heart attack or stroke)
I understand that My Prime Performance will take precautions to reduce these risks, but cannot eliminate them entirely.
Voluntary Participation
I voluntarily choose to participate in the services offered by My Prime Performance. I affirm that I am physically capable of engaging in these activities, or I have received appropriate clearance from my healthcare provider where necessary.
Assumption of Responsibility
I assume full responsibility for any injury, illness, or other health issue that may occur as a result of my participation, whether occurring during or after a session.
Waiver of Liability
I hereby waive, release, and discharge My Prime Performance, including its owner, employees, agents, and independent contractors, from any and all liability, claims, demands, or causes of action arising out of or related to my participation in any My Prime Performance program or service, to the fullest extent allowed by law.
Photo/Video Release (Optional)
I understand that I may be photographed or recorded during sessions for marketing or educational purposes. I will have the option to consent or decline this in the final form.
Agreement
I acknowledge that I have read this waiver in its entirety, understand it, and agree to the terms outlined. I also understand that I may ask questions before signing and can choose to withdraw from participation at any time.
